What will happen when I contact C.O.P.E. |
How to access C.O.P.E. Refarrals can come via:
Either phone or drop in and make and appointment. All new referrals are seen within 7 days sooner, if possible, in an emergency. First Appointment Usually lasts 30 minutes-1 hour although this can be longer depending on your needs. If you wish you can have a friend/other worker along with you for their first appointment. We will discuss a brief overview of C.O.P.E. and take an assessment of what you are looking for from the service. Initial goals set and the appropriate package of care identified. A return appointment is made usually within 7 days. Information packages are offered where appropriate e.g. advice on panic attacks. If something comes up and you cannot make your next appointment, just contact us and we will reschedule. We try at the first appointment to create a calm, relaxing, safe and reassuring environment to facilitate the individual feeling comfortable to return for support. Return appointment We will start a more detailed assessment of your needs and more fully explore care packages and how realistic the goals being set are as a measure of success in meeting your need. Also what preparatory work needs done before starting to work on achieving goals e.g. learning to relax, creating time to keep appointments Follow up appointments Usually on weekly or fortnightly basis with an option of using relaxation facilities between appointments. Packages begins to be worked through with regular reviews of progress against outcomes For some people a family member attends an appointment to gain a greater awareness of challenges and support loved one needs. Maybe they too need support which they would receive from a different worker. Discharge This happens when you have worked through your issues and achieved your goals. Resilience and coping strategies for future life challenges which are part of life’s rich tapestry are covered. If you need the service again in the future you can re refer even if it is only for a one off top up for reassurance. Dropping out/disengaging with service If you drop out or disengage we will contact you to find out why and what we can do to support re-engagement. Sometimes the person feels they do not need to return, we would remind them that the service is here and they can re refer some people decide that the time is not right to work through their issues, again we remind them when they feel differently we are here. |
